Clitoromegaly in pediatric and adolescent age group is usually indicative of a disorder of sexual differentiation. We report a girl child presenting with clitoral enlargement due to an epidermoid cyst. The cyst was excised with complete cosmetic recovery. Clitoromegaly or macroclitoris due to nonendocrinal causes is rare and rarer still is clitoral enlargement due to some underlying mass lesion, such as a pilonidal cyst, epidermoid cyst, or an abscess. A good clinical examination can spare the child from extensive investigations and associated emotional disturbances in such cases.

Periclitoral abscess is an uncommon problem in women without Female Genital Mutilation or piercing. There are only 18 cases reported in the literature recently reviewed by G. She had a 4 years history of recurrent periclitoral abscess, treated with incision and drainage in 3 occasions in other hospitals. This time she presented with 2 weeks history of swollen and pain in the clitoris hood, she was treated by her GP with Flucloxacilline and later with Metronidazole. Her general condition was good, being apyrexial.
